OWA: Your request couldn't be completed - Resolution with screenshots

This may have occurred for security reason or because your session timed out  - Resolution with screenshots

Situation: When accessing OWA or try to send email from OWA, you may receive this message:

Your request couldn't be completed. This may have occurred for security reason or because your session timed out.

Case 1: The client has Exchange 2010 and used to use http to access their OWA. After upgrading to SP3, they start receiving this message. For the security reason, https is required to access OWA. The resolution is install SSL and use https:.

Case 2: Quoted from Microsoft: If you’re using redirection for Outlook Web App and aren’t requiring Secure Sockets Layer (SSL), redirection will fail after the Client Access server is upgraded to Exchange 2010 SP2. To avoid this problem, after you’ve completed the upgrade to Exchange 2010 SP2, modify the Outlook Web App web.config file. For directions, go to “Use IIS Manager and Notepad to simplify the Outlook Web App URL when SSL isn’t required” in Simplify the Outlook Web App URL. You don’t have to make any changes in IIS Manager to prevent redirection from failing. You just have to modify the web.config file.

Case 3: Quoted from the Internet:

I did find the cause of my issue by reading that same article.  I needed to remove the Redirect inheritance on the virtual directories:

"When you configure redirection from a top-level directory in Windows Server 2008, the settings are propagated to lower-level directories. For example, when you configure redirection on the Default Web Site to the /owa virtual directory, the settings that you configure also appear on the HTTP Redirect page of all the virtual directories, such as /Autodiscover, /Exchange, and /Public. Therefore, you must remove redirection from all the virtual directories except the one that you want redirected."

I had already known this from previous experience, but forgot about it.  That's why I should have read the Release Notes first.
