iSCSI Initiator gets Authentication failure

Situation: when running iSCSI Initiator to access a target, you can see the target, but access. If you try, you may receive "Authentication failure" message.

Case 1: The client copied the iSCSI initiator node name to EqualLogic SAN, but missing the first letter, for example it should be iqn.1991-05.com.microsoft.pc1.chicagotech.net, but he copied only qn.1991-05.com.microsoft.pc1.chicagotech.net (missing i).

Case 2: Need to login Authenticate using CHAP user name.

Case 3: The computer's IP address is not in the accessing SAN IP range.
