SCSI: ASC MEDIA_LOAD_OR_EJECT_FAILED

Symptoms: The client is running Symantec Backup exec 2010 with Exabyte Storage. The Device shows Offline and one of tapes stuck in the device. The device also shows in use randomly. The Symantec Backup Exec Adamm.log and Tracer.exe show this message:
